These Website Terms of Use (“Terms”) govern your access to and use of www.renovapsychiatriccare.com (the “Website”). By accessing or using the Website, you acknowledge that you have read, understood, and agree to these Terms. If you do not agree, please discontinue use of the Website.

These Terms apply only to your use of the Website. They do not replace or modify any separate clinical agreements, informed consents, telehealth consents, financial policies, privacy notices, treatment documents, or other documents that may apply to services provided by Renova Psychiatric Care.

1. NO PROVIDER-PATIENT RELATIONSHIP

Use of this Website—including browsing, reading information, submitting an appointment request, sending an email, or otherwise communicating electronically—does not by itself establish a provider-patient relationship.

A provider-patient relationship is established only after Renova Psychiatric Care completes its applicable intake, evaluation, consent, eligibility, and other required processes and agrees to provide clinical services.

Submitting an appointment request does not guarantee an appointment or acceptance as a patient. Renova Psychiatric Care reserves the right to determine whether services are clinically appropriate and whether the practice can accept an individual for care.

2. EMERGENCIES AND CRISIS COMMUNICATIONS

THIS WEBSITE, EMAIL, CONTACT FORMS, AND GENERAL ELECTRONIC COMMUNICATIONS ARE NOT MONITORED FOR EMERGENCIES.

Do not use the Website or general electronic communications to report a medical or psychiatric emergency, suicidal crisis, imminent risk of harm, or any situation requiring immediate assistance.

If you are experiencing an emergency or believe that you or another person is in immediate danger:

• Call 911;

• Call or text 988 for the Suicide & Crisis Lifeline; or

• Go to the nearest hospital emergency department.

Renova Psychiatric Care does not guarantee that emails, contact forms, portal messages, or appointment requests will be reviewed or answered immediately.

3. TELEPSYCHIATRY SERVICES

Renova Psychiatric Care may provide psychiatric services through telehealth when clinically appropriate and permitted by applicable law, professional requirements, payer requirements, and practice policies.

Telehealth services may require verification of patient identity and physical location, assessment of clinical appropriateness, informed consent, compliance with applicable licensure and jurisdiction requirements, and appropriate privacy, technology, and emergency procedures.

Renova Psychiatric Care may determine that telehealth is not appropriate for a particular patient, visit, condition, or circumstance and may recommend an in-person evaluation, emergency evaluation, referral, or another appropriate level of care.

4. PATIENT LOCATION AND JURISDICTION

Patients receiving telehealth services may be required to provide their current physical location at the time of an appointment.

Telehealth services may be subject to the laws and professional requirements of the jurisdiction in which the patient is physically located when services are provided. Renova Psychiatric Care may be unable to provide services when a patient is located outside a jurisdiction in which the practitioner is authorized to provide services.

Renova Psychiatric Care currently provides telehealth services to patients located in New York State, subject to applicable requirements.

Patients should notify the practice before an appointment if they expect to be physically located outside New York State.

5. TELEHEALTH TECHNOLOGY

Telehealth services depend on internet connectivity, compatible devices, software, audio/video capabilities, and other technology.

Renova Psychiatric Care does not guarantee that telehealth technology will operate without interruption, delay, error, or technical difficulty. Technical problems may result in a delayed, interrupted, rescheduled, converted, or discontinued appointment when clinically appropriate.

Patients are responsible for having a compatible device, adequate internet access, and an appropriate environment for participating in telehealth services.

Patients are encouraged to participate from a private location and to use reasonable security measures, including password-protected devices and secure internet connections when available.

6. SIMPLEPRACTICE AND THIRD-PARTY TECHNOLOGY

Renova Psychiatric Care uses SimplePractice and may use other third-party technology platforms and service providers to support practice operations, including scheduling, patient intake, electronic health records, patient portal communications, telehealth, billing, payment processing, and document exchange.

Use of third-party platforms may be subject to the third party’s own terms, privacy policies, consent requirements, and other applicable policies.

Renova Psychiatric Care does not control the operation or availability of third-party services and is not responsible for third-party failures except as required by applicable law.

7. ELECTRONIC COMMUNICATIONS

The Website may permit you to contact Renova Psychiatric Care by telephone, email, or an appointment-request form.

Current patients should use the designated secure patient portal or other communication method provided by Renova Psychiatric Care for clinical communications when appropriate.

Please do not submit unnecessary protected health information, medication information, psychiatric information, or other sensitive clinical information through a general Website contact form or unsecured email.

Electronic communications may be delayed, interrupted, misdirected, or inaccessible. Renova Psychiatric Care does not guarantee a particular response time.

8. WEBSITE INFORMATION AND CLINICAL DISCLAIMER

Renova Psychiatric Care makes reasonable efforts to provide accurate and current information on the Website. However, Website content may become outdated or contain errors or omissions.
